Exam Accommodation Request Form

This form is for faculty members who wish OUE to provide exam space and proctoring for students with DAS accommodations

Accommodated Final Exam Request form

This form is for faculty members who wish OUE to provide final exam space and proctoring for students with DAS accommodations.

Proctored Final Exams (Non-accommodated exams)

This form should be used by Emory College faculty only to request proctored exam make-ups for students who are unable to take their in-person exam at the regularly scheduled time. OUE does not provided proctored make-up exams for online courses. 

This form is for regular exams only. Please use the Accommodated Final Exam form for students with DAS accommodations.

 

Retention Alert Form
This form is designed for use by the Emory community to notify OUE about students who may be considering transferring. 
Student Concern Form

This form is designed for faculty, staff and administrators to notify university administrators of any concerns they have regarding particular students.

Final Exam Policy

Student Absences: Guidelines for Faculty

Students often request "excused" absences from class. OUE does not excuse routine absences. This document outlines circumstances in which OUE will work with students and instructors, and clarifies the role and responsibilities of faculty in handling absences.

OUE Syllabus Add

This one-page addendum for syllabi provides information about academic deadlines, policies, and resources. Faculty may include this with their syllabi or incorporate or adapt specific sections as they see fit. The document is updated each semester and posted before classes begin.

Honor Code Resources for Faculty

This link will direct faculty to resources and College policy on dealing with issues of academic misconduct.

Religious Holiday Calendar

The Office of Spiritual and Religious Life maintains a calendar of the major Christian, Jewish and Islamic holidays that will occur during university term dates. This resource may assist faculty in course planning.

This resource is designed to support DUS and Undergraduate Staff members complete the appropriate paperwork to submit new courses to the College Curriculum Committee.
